1. In Theatre book a theatre, time, procedure and side, surgeon and anaesthetist — two operations never share a theatre at once.
2. The anaesthetist writes the pre-anaesthetic check (ASA); the consent — who gave it, and the witness.
3. The WHO surgical safety checklist — Sign In, Time Out, Sign Out — in order, each answered before the next opens.
4. The operation and anaesthesia notes signed; recovery scores, then to a ward, ICU or home.
